Please list at least 5 things you would like to get out of coaching with me. Please give as much detail as possible.
If so, what kept you from reaching them, or what made you revert? If not, what has prompted you to start now?
What do you feel worked well? What did you feel didn't work well? Which parts of it did you like or not like?
For example, a boost in confidence and improved self-perception.
10 being it is your foremost priority and you are willing to make significant sacrifices and dedicate a large amount of time and focus to it. 1 being it is not something you are willing to put a large amount of time or focus into. There is no right or wrong answer to this. If your career or family take a higher priority, it is okay to put a lower number on this scale.
This can be anything; family, job, hobbies, etc.
What kind of exercise have you done in the past? What did you like? What didn't you like?
